Please note because of space and availability this is a preregistration form. You will be notified via separate email by August 21st if your spot in Truth Builders has been approved.  Once your spot is approved you will receive additional links for payment through Zeffy. 1. I authorize the staff and volunteers of Old North Church (ONC) to seek emergency medical attention for my child if needed, with the understanding that every effort will be made to contact me as soon as possible. If I cannot be reached, I consent to any immediate and necessary medical treatment — including, but not limited to, injection, anesthesia, or surgery — as deemed advisable by a licensed medical professional. I understand that ONC and its representatives will not be held liable for any accident or injury in connection with such emergency care.


2. I understand and agree that ONC will take reasonable precautions to ensure a safe environment, but that participation in Kids Ministry events for the 2025–2026 church year involves inherent risks. I release and hold harmless ONC, its staff, and volunteers from any liability for injuries sustained in connection with these events.


3. I give permission for my child’s image to be photographed or recorded during ONC activities and used — without personal identification — in ONC publications, promotional materials, social media, and on the church website. I understand that these images may be used without further notice or compensation.


4. I understand the class offerings, age guidelines, and associated program fees, and that payment must be received before materials are provided and participation is confirmed.
